When we first meet you, we will ask questions about your health, identify your concerns, help you set goals, and discuss the next steps to take toward achieving them. Our services, tailored specifically for you, may include one or more of the following:

  • Develop a customized care plan

  • Accompany you to doctors’ appointments and take notes for you

  • Review and coordinate your medications

  • Review your medical records

  • Create your medical history timeline

  • Facilitate communication between you and your healthcare providers

  • Speak with healthcare providers and family members on your behalf

  • Interpret complicated medical information and terminology

  • Coordinate appointments (lab, radiology, doctors, etc.)

  • Develop a list of questions for you to ask your providers

  • Coordinate with other professionals who can help when needed
    (social workers, nursing home or assisted living administrators)

  • Research options for medical treatment

  • Research options for transitions of care

  • Manage care for your loved one who lives elsewhere

  • Find community-based resources that may be available

  • Provide consultation for advanced care planning

We guide and assist clients in medical decision making; however, at no time will we make decisions about your health or medical care. We do not provide hands-on care, and we do not provide transportation for you or your family members.
